Nationwide Insurance® Woman of Courage Nomination Rules

 


These are the Official Rules of the Nationwide Insurance® Woman of Courage Award nomination process (“Process”), and supersede all other documents.  The Award and Process are sponsored by Nationwide Mutual Insurance Company (“Nationwide”), One Nationwide Plaza, Columbus, Ohio 43216.

 

No purchase or insurance quote is necessary to nominate a Woman of Courage, or to be selected as a Woman of Courage Award recipient. 

 

ELIGIBILITY

Both nominees and nominators must be legal residents of the fifty United States and the District of Columbia who: (a) are 18 years of age, and (b) are legally able to work in the United States. 

 

Potential Nationwide Insurance® Woman of Courage Award recipients must be willing and able to travel to the Los Angeles area for the program taping.  Recipients and Recipient’s Guest (his/her parent/legal guardian if a resident of a jurisdiction that deems him/her to be a minor) will be required to execute, sign, have notarized and return an Affidavit of Eligibility, a Liability Release, a Publicity Release (where legal) and any other documentation required by Nationwide, or that potential recipient may be disqualified and an alternate recipient may be selected at Nationwide’s sole discretion.

 

AWARD SELECTION

The 2008 Woman of Courage Award recipient or recipients will be a cancer survivor or cancer advocate, selected from nominations received.

 

To nominate a Woman of Courage, submit an essay of 250 words or less regarding the nominee’s contributions and why she deserves to be the 2008 Nationwide Insurance Woman of Courage.

 

Nationwide and its television partners, in their sole discretion, will select an Award recipient or recipients based on nomination essays and other information learned about the nominees.  Nationwide reserves the right to conduct a background investigation into any potential recipient. 

 

RECIPIENT(S)

Some or all recipient(s) will be announced during the airing of the “Frosted Pink with a Twist” television program, tentatively scheduled to air on the ABC television network on October 12, 2008. 

Nationwide, Edge Entertainment, Inc., their its parent companies, subsidiaries, affiliates, advertising/promotion agencies, and anyone else connected with the production and distribution of this Process (hereafter, collectively "Released Parties") shall not be liable for failure of the program to air as scheduled, or if some recipients are not announced or honored on air. 

 

Recipient(s) and a guest will travel to the Los Angeles area, at Nationwide’s expense, for the taping of theFrosted Pink with a Twist” television program. 

 

GENERAL RULES

The Process is governed by and subject to the laws of the United States. All federal, state and local laws and regulations apply. Void where prohibited by law. Recipient(s) and their guest(s) may receive an IRS 1099 form for the value of his or her travel expenses. By participating in the Award nomination process, nominators and nominees grant the Released Parties permission to use their name, likenesses, and image in connection with the Award for purposes of advertising, publicity and promotion purposes, without further compensation to the entrant, unless prohibited by law.

 

By participating in the Process, nominators and nominees agree to release and hold harmless the Released Parties from and against any claim or cause of action arising out of participation in the Process or receipt or use of any Award.

 

Entry constitutes consent to participate in the Process and for the Release parties to obtain, use, and transfer the applicant and nominee’s name, address and other details for Award evaluation and administration purposes.

The Released Parties will not be responsible for lost, incomplete, illegible, late, misdirected, stolen, or mutilated entries; technical or mechanical malfunctions; errors in the Official Rules, in any related ads or materials; or processing error. 

 

Entries must contain only original content by nominators and may not include content of any other third parties.  Entries may not violate any applicable laws and regulations. Entries may not contain any profanity or sexually explicit content. Entries may not defame or invade the rights of privacy of any person, living or dead, or otherwise infringe upon any third party’s personal or proprietary rights. Entries that, in the sole discretion of Nationwide, fail to meet any of the criteria set forth above or contain any objectionable content will be disqualified.

 

All Entries become the property of the Nationwide. As a condition for participating in the Process, Nominators irrevocably assign and transfer all rights, title and interest in and to their Entries, including but not limited to copyrights and all other proprietary rights, to Nationwide. By participating in the Process, nominators further agree that additional written authorization, assignment or other documentation requested by Nationwide in connection with the transfer and assignment of rights to their Entries may be required as a condition of entry and agree to execute any such documents at the request of Nationwide.

 

Nationwide obtains the right, without limitation, to use, edit, and publish, in whole or in part, any and all Entries in any media whatsoever, without time or geographical restrictions, without providing any additional compensation or consideration to nominators.
